Destekli Çatlatma (Assisted Hatching)

Destekli çatlatma, bazen in vitro fertilizasyon (IVF) tedavisi ile birlikte insan erken embriyolarına uygulanan bir laboratuvar prosedürüdür. IVF, yumurtaların spermle laboratuvar ortamında karıştırılması işlemidir (doğal gebelikte olduğu gibi fallop tüpünde değil). IVF sırasında döllenmiş yumurtaların 3 ila 6 gün boyunca bölünmeleri ve embriyolara dönüşmeleri izlenir. En iyi embriyo seçilerek gebelik oluşturma umuduyla rahme transfer edilir (embriyo transferi) veya daha sonraki kullanımlar için dondurulur. Yumurta, gelişimi sırasında onu koruyan zona pellucida adlı bir dış kabukla çevrilidir. Embriyo büyüdükçe bu kabuğu doğal olarak kırarak dışarı çıkar. Bazen doktor, embriyo rahme transfer edilmeden hemen önce IVF laboratuvarından embriyonun dış kabuğunda küçük bir “çatlak” veya “delik” açmasını isteyebilir (bu işleme destekli çatlatma denir). Destekli çatlatmanın, embriyonun dış kabuktan çıkmasına, genişlemesine, rahim duvarına tutunmasına ve sonuçta gebelik oluşturmasına yardımcı olabileceği umulmaktadır.

Destekli çatlatma nasıl yapılır?

Destekli çatlatma sırasında embriyonun dış kabuğu, mikroskop altında zona pellucida’ya küçük bir delik açılarak yapay olarak zayıflatılır. Bu işlem birkaç farklı yöntemle gerçekleştirilebilir. En yaygın yöntem, kabuğu “çatlatmak” için lazer darbeleri kullanmaktır. Daha eski ve nadir kullanılan bir yöntem ise Tyrode solüsyonu adı verilen asidik bir çözeltinin uygulanarak kabukta küçük bir delik açılmasıdır.

Destekli çatlatma embriyoya zarar verebilir mi veya ileride gebelikte sorun yaratır mı?

Nadiren de olsa, destekli çatlatma embriyoya zarar verebilir ve embriyoyu kullanılmaz hale getirebilir. Destekli çatlatmanın uygulandığı durumlarda tek yumurta ikizi riski biraz artabilir.Tek yumurta ikizlerinde tıbbi komplikasyonlar, normal tekil gebeliklere göre daha fazladır.

Destekli çatlatmadan ben fayda görür müyüm?

Uzmanlar, IVF tedavisi gören tüm hastalara destekli çatlatma uygulanmasını önermemektedir. Bazı çalışmalar, belirli hasta gruplarında destekli çatlatmanın
gebelik şansını artırabileceğini göstermektedir. Daha önceki IVF döngülerinde gebelik elde edememiş olan veya gebe kalma ihtimali düşük olan hastalarda, destekli çatlatma gebelik şansını artırabilir. Fertilite uzmanınız, destekli çatlatmanın sizin için uygun olup olmadığını belirlemenize yardımcı olabilir.

Destekli çatlatma yapılmasının başka nedenleri var mı?

Eğer preimplantasyon genetik test (PGT) planlanıyorsa, döllenmeden sonraki üçüncü günde embriyoların biyopsisi için destekli çatlatma yapılabilir. Biyopsi
sırasında, döllenmeden sonraki beşinci veya altıncı günde (blastokist evresi), embriyonun dış hücre tabakasından (trofektoderm) az sayıda hücre alınır.
