Fertility and Sterility Publishes Editorial Exploring the Origins of “Restorative Reproductive Medicine” and Why Modern Fertility Care Must Remain Comprehensive

In “A Personal Professional Journey: The Real History of Restorative and Responsible Reproductive Medicine,” Dr. Rebar reflects on five decades of experience in reproductive endocrinology to show that the principles underlying RRM—identifying and treating root causes of infertility—have long been part of standard fertility care. He cautions that presenting RRM as a distinct or superior alternative overlooks the role of male-factor infertility and the essential advances made possible by in vitro fertilization (IVF) and other assisted reproductive technologies (ART).
Dr. Rebar explains that before the advent of IVF in the late 1970s, options for patients with certain conditions such as blocked fallopian tubes or severe male infertility were extremely limited. The development of IVF, intracytoplasmic sperm injection (ICSI), and related technologies fundamentally changed what was possible for patients, shortening the time to pregnancy and expanding opportunities for families.
The editorial also underscores that the medical subspecialty of Reproductive Endocrinology and Infertility (REI) was explicitly founded to ensure comprehensive, responsible, and science-based care for patients facing infertility and related conditions.
“I believe that what is now being termed restorative reproductive medicine, aimed at identifying and treating the root causes of infertility, represents a step backwards to the practice of infertility in the 1960s and 1970s,” writes Dr. Rebar. “Skilled clinicians who are trained and Board-certified as Reproductive Endocrinology and Infertility subspecialists carefully consider all possible causes of infertility in both the female and the male partners and recognize that many infertile individuals have more than one cause for their problem. Up-to-date clinicians utilize all tools now at our disposal to appropriately manage infertility.”
Fertility and Sterility is the flagship publication of The American Society for Reproductive Medicine (ASRM). The peer-reviewed journal runs internationally and is published monthly.
